Home Health Physical Therapist - Full Time

Position Overview Seeking a Full-Time Physical Therapist (PT) to provide home health care services in the South/Central Hillsborough County area. The role involves enabling patients to recover from illness or injury at home.

Key Responsibilities

• Include Conducting initial and ongoing assessments of mobility, strength, and function.
• Administering and interpreting tests to set functional goals and outcomes.
• Developing and updating detailed plans of care, including progress tracking and discharge planning.
• Delivering therapy services per physician orders, documenting effectively, and ensuring compliance with policies.
• Collaborating with healthcare team members, educating patients and caregivers, and supervising clinical personnel as applicable.
• Completing OASIS assessments within scope of practice and adhering to regulations and standards.
• Qualifications & Skills Degree in Physical Therapy from an accredited program.
• Unrestricted state license as a Physical Therapist.
• Valid CPR certification.
• Preferred: strong organization, communication, attention to detail, time management, and experience in home health therapy and OASIS documentation.
