Home Health Physical Therapist - Full Time

Position Overview Seeking a Physical Therapist (PT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Collaborates with interdisciplinary teams to optimize patient well-being through evaluations, treatment plans, and progress documentation.

Key Responsibilities

• Perform initial and ongoing assessments of mobility, strength, and function.
• Administer tests, interpret results, and set functional goals.
• Develop and update plans of care, including discharge planning and patient education.
• Deliver therapy services per physician orders, maintaining thorough documentation.
• Coordinate with healthcare team members, patients, and caregivers.
• Supervise applicable personnel (PTA, HHA, OTA) and complete assessments like OASIS where permitted.
• Qualifications & Skills Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR, driver’s license, and auto insurance.
• Experience in home health therapy and OASIS documentation preferred.
