Supportive Care Specialist Group is seeking a compassionate Palliative Nurse Practitioner to join our growing clinical team serving primarily geriatric patients in hospital and LTAC settings. This role offers strong clinical autonomy, a supportive multidisciplinary environment, and the opportunity to make a meaningful impact on patients and families.

Compensation & Benefits

• Starting at $155,000 annually + productivity potential

Responsibilities

• Daily rounding split between acute care hospital and LTAC
• Provide palliative care primarily for geriatric patients
• Support patients and families through complex care decisions
• Collaborate with physicians and interdisciplinary teams
• Maintain accurate clinical documentation

Qualifications

• Active Family Nurse Practitioner license (California)
• Minimum 2 years inpatient hospital experience + 2 years primary care experience
• Strong communication skills and compassionate bedside manner
• Ability to work independently with a patient-centered approach
• Vietnamese speaking is a plus

Training &

Support

• Structured onboarding and clinician shadowing
• Ongoing supervision from board-certified palliative physicians
• Support staff including social workers, chaplains, and office team assistance
