About the Role

Supportive Care Specialists is hiring a Palliative Care Nurse Practitioner in Baldwin Park, California. You’ll assess patients with serious illness, address their symptoms and care goals, and work closely with patients, families, and healthcare professionals. Thoughtful symptom management and coordinated care help patients and families make informed decisions and stay comfortable.

This role calls for compassionate communication, sound clinical judgment, and clear, accurate documentation.

What You’ll Do

• Perform patient assessments and physical exams, and review medical records and relevant diagnostic information
• Develop and adjust care plans, manage acute and chronic symptoms, and prescribe medications in line with applicable regulations and collaborative practice agreements
• Coordinate with physicians, nurses, hospice teams, and other providers on referrals, transitions, and discharge planning
• Document care in the EMR/EHR while protecting patient privacy under HIPAA

What You Need

• Active California Nurse Practitioner license with applicable prescribing authority
• Strong patient assessment, communication, medical documentation, and medication-management skills

Nice to Have

• Experience in hospice and palliative medicine, geriatrics, senior care, home care, nursing home, assisted living, hospital, acute care, or post-acute care
• EMR/EHR experience (Epic, Cerner, Athenahealth, or eClinicalWorks)
• Knowledge of medical terminology, anatomy, physiology, and ICD coding
• Experience in triage, case management, pain management, behavioral health, or care for people with disabilities
