Home Health Occupational Therapist - Full Time

Position Overview

We are seeking a

Full-Time Occupational Therapist (OT)

to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves performing initial and ongoing evaluations of patients' mobility, strength, and function, and providing detailed plans of care with goals and outcomes.

Responsibilities include administering tests, documenting progress, modifying treatment plans, and collaborating with healthcare team members, patients, and caregivers. The OT will also handle discharge planning, educate families on home exercises, supervise relevant personnel, and complete assessments such as OASIS where permitted.

Requirements:

• Degree in Occupational Therapy from an accredited program
• Current, unrestricted state license
• Valid CPR certification
