Physical Therapist (PT) – Home Health

Visiting Nurses of the Lower Valley, Inc. is a not-for-profit, Medicare- and Medicaid-certified home health care organization recognized among the nation’s best for home health quality and performance.

We are seeking an experienced Physical Therapist (PT) for an immediate per diem or part-time opportunity serving Essex, Connecticut, and surrounding communities.

Position Responsibilities

• Perform initial and ongoing patient assessments to determine functional abilities and rehabilitation needs.
• Develop individualized plans of care in coordination with physician orders.
• Provide skilled physical therapy services to help patients improve mobility, strength, balance, and overall functional independence.
• Monitor patient progress and adjust treatment plans as appropriate.
• Maintain timely, accurate, and thorough clinical assessments, progress notes, and other documentation in accordance with professional standards and organizational policies.

Qualifications

• Graduate of an accredited college or university with a degree in Physical Therapy.
• Current, valid Connecticut Physical Therapist license.
• At least one year of physical therapy experience in a home health care setting.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently while collaborating with patients, families, physicians, and the interdisciplinary care team.

Compensation

• Competitive pay: $75 per visit (not per hour).
