(1099 Contracted Position with potential to grow into full-time salaried position in the near future if interested)

We are looking for qualified Physical Therapists to become a part of our rapidly growing home health agency. All are encouraged to apply! Home health experience highly preferred- willing to train the right candidate.

We want therapists who care about what they do and have strong attention to detail. Willing to learn home health specific documentation.

Duties & Responsibilities

• Provide skilled physical therapy services in accordance with physician orders
• Perform initial evaluations, OASIS assessments (as applicable), and ongoing reassessments
• Develop, implement, and update individualized treatment plans in collaboration with the physician and care team
• Ability to utilize home health-specific EMR software. We use Finale Health- which also includes and app for utilizing in the field.
• Deliver therapeutic interventions to improve mobility, strength, function, and pain management
• Educate patients and caregivers on home exercise programs, safety, and use of assistive devices (walkers, canes, braces, etc.)
• Monitor patient progress and communicate changes in condition to the physician and interdisciplinary team
• Supervise Physical Therapist Assistants (PTAs) per agency policy
• Complete accurate and timely documentation within 72 hours of each visit
• Participate in care coordination, case conferences, and discharge planning

Qualifications

• Graduate of an accredited Physical Therapy program
• Active Nevada PT license
• Minimum 1 year of home health experience preferred
• Current CPR certification (AHA or Red Cross with in-person skills validation)
• Valid driver’s license and reliable transportation
• Strong clinical, communication, and documentation skills

Job Requirements

• Ability to travel to patient homes
• Physical ability to perform patient care, including lifting up to 30 lbs and assisting with mobility
• Must be able to effectively communicate in English (bilingual a plus)

Pay: $90.00 - $130.00 per hour
