In this flexible position, you will evaluate, develop, and implement personalized treatment plans to promote mobility, reduce pain, and improve overall quality of life for patients of all ages.

Your expertise will support our patients in our skilled nursing facility, ensuring seamless recovery journeys for those in need. This role offers the opportunity to work independently while collaborating closely with a multidisciplinary team to achieve optimal patient outcomes.

Responsibilities

  • Conduct comprehensive assessments of patients’ physical conditions, including mobility, strength, and functional abilities.
  • Develop and modify individualized care plans based on patient needs, clinical standards, and treatment goals.
  • Deliver evidence-based physiotherapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and modalities tailored to each patient’s condition.
  • Document patient progress accurately in electronic health records (EHR) systems following clinical documentation standards.
  • Collaborate with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to coordinate care plans and discharge strategies.
  • Educate patients and their families on treatment techniques, home exercises, and community resources to support ongoing recovery.
  • Manage discharge planning processes by assessing patient needs for continued therapy or community support services.

Skills

  • Strong knowledge of physiology, anatomy, orthopedics, neurology, geriatrics, pediatrics, and related medical terminology.
  • Proven experience practicing in various physical therapy roles across inpatient hospital settings, outpatient clinics, home health environments, or rehabilitation centers.
  • Familiarity with skilled nursing facilities.
  • Ability to develop effective treatment plans that align with clinical standards and patient goals.
  • Proficiency in electronic health records (EHR) management and EMR systems for accurate documentation of patient care activities.
  • Excellent client communication skills to engage geriatric and pediatric populations—and foster a positive therapeutic environment.
  • Experience with discharge planning processes and working within skilled nursing facilities to ensure smooth transitions from hospital to home or community care.
  • Knowledge of medical terminology relevant to physiotherapy practices in skilled nursing facilities.
  • Ability to work collaboratively with healthcare teams to optimize patient rehabilitation outcomes.

Join us in making a meaningful difference through compassionate physical therapy! This position offers flexible scheduling options designed to fit your lifestyle while allowing you to provide vital rehabilitative services across diverse clinical environments.
