About this role

Service Area: Both Horry and Georgetown Counties (entire counties)

When: On-call coverage every other weekend, Friday at 4:30PM through Monday at 8:30AM

Why Work With Tidelands Community Hospice

At Tidelands Community Hospice, caring for patients and families is more than the work we do. Since 1985, we have provided compassionate, personalized hospice care to individuals with life-limiting illnesses and their families. As the only locally based and governed, not-for-profit hospice provider serving Georgetown, Horry, and Williamsburg Counties, we are rooted in a tradition of neighbors helping neighbors.

We are looking for an experienced Registered Nurse to provide weekend on-call coverage and compassionate, clinically sound care to patients and families throughout both Horry and Georgetown Counties. You will independently respond to patient and caregiver needs, including providing in-person nursing visits, admissions, symptom management, and end-of-life care as needed.

You will work closely with patients, families, physicians, and our interdisciplinary team to provide comfort, coordinate care, and support patients and families throughout the end-of-life journey.

What You’ll Do

  • Respond to hospice patient and family care needs, including nursing visits, admissions, and other clinically necessary services
  • Perform nursing assessments and monitor changes in patient condition
  • Develop, implement, and update individualized plans of care
  • Provide skilled nursing care, symptom management, and comfort-focused interventions
  • Coordinate care with physicians, caregivers, facilities, and the interdisciplinary hospice team
  • Educate and support patients and families throughout the hospice care journey
  • Maintain accurate, timely, and confidential clinical documentation
  • Independently organize and prioritize patient visits, admissions, and time-sensitive clinical needs
  • Independently travel throughout both Horry and Georgetown Counties to provide in-person care in patients’ homes, facilities, and other care settings as needed
  • Provide scheduled every-other-weekend on-call coverage, responding to patient and caregiver calls, nursing visits, admissions, deaths, and other clinical needs as they arise

What We’re Looking For

Required:

  • Graduate of an accredited school of professional nursing
  • Associate degree in nursing (ADN)
  • Current, valid South Carolina Registered Nurse (RN) license
  • Current, valid driver’s license
  • Current, valid automobile insurance
  • Minimum of one year of employment experience as a Registered Nurse
  • Minimum of one year of hospice, palliative care, home health, skilled nursing, long-term care, or other comparable experience providing ongoing patient-centered nursing care
  • Experience performing patient assessments, developing and implementing plans of care, and monitoring changes in patient condition
  • Experience providing symptom management, comfort-focused care, medication education, and patient and caregiver support
  • Ability to independently prioritize patient needs, respond to urgent clinical situations, and exercise sound clinical judgment
  • Strong interpersonal and communication skills with the ability to communicate compassionately and effectively with patients, families, caregivers, physicians, and interdisciplinary team members
  • Strong documentation, organization, time-management, and follow-through skills
  • Ability to work independently in a community-based setting while collaborating effectively as part of an interdisciplinary care team
  • Ability to independently travel throughout both Horry and Georgetown Counties to patients’ homes, facilities, and other care locations as needed
  • Ability to provide scheduled on-call coverage every other weekend, Friday at 4:30PM through Monday at 8:30AM, and respond to patient care needs as they arise

Preferred:

  • Bachelor of Science in Nursing (BSN)
  • Certified Hospice and Palliative Nurse (CHPN®) certification is a plus
  • Experience providing nursing care in patients’ homes or other community-based settings
  • Experience working as part of an interdisciplinary hospice or healthcare team
  • Experience supporting patients and families through end-of-life care
  • Experience with electronic medical record (EMR) or electronic health record (EHR) systems
  • Strong knowledge of hospice philosophy, symptom management, comfort-focused care, and end-of-life nursing practices

Work Schedule & Environment

  • Part-time, every-other-weekend on-call position
  • Scheduled on-call coverage is Friday at 4:30PM through Monday at 8:30AM, every other weekend
  • No regularly assigned patient caseload or scheduled routine patient visits; clinical services are provided as needed, including patient and caregiver calls, nursing visits, admissions, deaths, and coordination with physicians and other members of the care team
  • Regular and reliable availability during assigned on-call periods is required. Time off during assigned weekends is expected to be limited, requested in advance, and approved based on coverage needs
  • On-call coverage may be provided from the RN's residence or another appropriate location when not actively responding to patient care needs
  • Respond to patient and caregiver needs using appropriate clinical judgment, including conducting in-person visits when clinically indicated rather than relying solely on telephone consultation or intervention
  • Regular independent travel throughout both Horry and Georgetown Counties is required as needed to provide in-person nursing care in patients' homes, healthcare facilities, and other care settings
  • Work environments and conditions may vary based on patient location and care needs
  • Regular interaction with patients experiencing serious or terminal illness and with patients and families experiencing end-of-life circumstances is an inherent part of the position

Compensation & Benefits:

  • Paid time off
  • 401(k) retirement plan with a 2% employer contribution (pending eligibility confirmation)
  • Mileage reimbursement for required work-related travel
