About the Role

Supportive Care Specialist Group is hiring a compassionate, experienced Palliative Care Nurse Practitioner to care for adult and geriatric patients with serious illness in San Diego hospitals and Long-Term Acute Care (LTAC) facilities. You’ll work with a high degree of clinical independence as part of a collaborative, multidisciplinary team, helping patients and families make important healthcare decisions.

Responsibilities

• Round daily at hospital and LTAC facilities
• Provide palliative care consults and manage complex symptoms
• Lead goals-of-care conversations with patients and families
• Collaborate with attending physicians, nursing staff, and interdisciplinary teams
• Develop individualized care plans
• Complete timely, accurate clinical documentation

Required Qualifications

• Active California Nurse Practitioner license
• 2+ years of inpatient hospital experience
• 2+ years of primary care experience
• Excellent communication skills and a compassionate bedside manner

Preferred Qualifications

• Experience in palliative care, hospice, or geriatrics
• Bilingual in Spanish

• On-site rounding at hospital and LTAC facilities
