About this role
A Plus Healthcare at Home is expanding home health services in Park City and the Wasatch Back, and we're looking for a PRN Physical Therapist who lives in or near Park City, Heber City, Midway, Kamas, or Coalville. Treat patients in your own community, set your own availability, and skip the canyon commute.
Many of our patients are coming home after joint replacements, ACL repairs, fractures, and other orthopedic surgeries, so your work will have a direct, visible impact on getting active people back on the trails, slopes, and golf course.
Position Highlights
- PRN / flexible schedule, you help set your availability
- Visits in Park City and surrounding Summit and Wasatch County communities
- Work close to home instead of commuting to the Salt Lake valley
- Pay: negotiable based on experience
- $0.50/mile reimbursement
- One on one treatment time in the patient's home, without the pace of a busy clinic
- Great fit for outpatient or hospital PTs looking for extra income, or parents wanting flexible hours
- Opportunity to grow into a full-time position as the territory grows
What You'll Do
- Perform PT evaluations and reassessments in the home
- Develop and carry out individualized treatment plans with the physician and care team
- Provide therapeutic exercise, gait and transfer training, balance work, and post-surgical rehab
- Assess home safety and recommend equipment or modifications
- Teach patients and caregivers home exercise programs and fall prevention
- Complete timely, accurate documentation
We're Looking For Someone Who
- Holds a current Utah Physical Therapist license (or PT Compact privilege) and CPR certification
- Has strong evaluation and clinical reasoning skills
- Is comfortable working independently in the home setting
- Has a valid driver's license, reliable vehicle, and auto insurance (winter driving experience a plus)
- Home health experience is preferred but not required. Outpatient ortho, sports medicine, acute care, and SNF therapists are encouraged to apply.
