About Our Practice

Made to Thrive Mental Health Wellness is a growing, women-focused healthcare practice in Fort Wayne, Indiana. We specialize in caring for women through every stage of life, with an emphasis on mental health during adolescence, pregnancy, postpartum, perimenopause, and menopause.

We are excited to expand our services to include hormone replacement therapy (HRT) and menopause management.

We are seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our team and help grow this new area of care.

About the Position

This is a flexible, part-time, hybrid opportunity for a provider who is passionate about women's health and interested in helping women navigate perimenopause, menopause, and hormonal changes.

Experience with hormone replacement therapy is preferred, but we are willing to provide additional training and mentorship to the right candidate.

We welcome applicants from a variety of clinical backgrounds, including Family Nurse Practitioners (FNP), Women's Health Nurse Practitioners (WHNP), Certified Nurse-Midwives (CNM), and Physician Assistants (PA).

Responsibilities

• Evaluate and treat women experiencing perimenopausal and menopausal symptoms.
• Develop individualized, evidence-based hormone therapy treatment plans.
• Prescribe and monitor hormone replacement therapy within the provider's scope of practice.
• Order and interpret appropriate laboratory testing.
• Provide education regarding hormonal health, treatment options, and lifestyle modifications.
• Collaborate with psychiatric providers to support women's overall mental and physical wellness.
• Participate in developing clinical protocols and workflows for our expanding hormone health services.
• Maintain accurate and timely documentation.

Qualifications

Required:

• Licensed or eligible for licensure as an NP, CNM, or PA in Indiana.
• Appropriate national certification.
• Ability to obtain required prescriptive authority and meet applicable Indiana practice requirements.
• Strong clinical judgment, communication skills, and commitment to patient-centered care.
• Genuine interest in women's health and hormone management.

Preferred but Not Required:

• Experience in women's health, gynecology, primary care, or menopause management.
• Previous experience prescribing and managing hormone replacement therapy.
• Specialized menopause training or certification.
• Experience in outpatient or private practice.
