Home Health Occupational Therapist

Grace Community Home Health is a Christian/Faith-Based Agency seeking a Per Diem Occupational Therapist who has a desire to provide excellent care, and wants to be part of a team who loves to serve our community.

Occupational Therapist Home Health Position Summary

• Full independence and schedule flexibility
• Independently evaluate and conduct follow-up visits as an Occupational Therapist
• Provide Occupational Therapy services to clients in their home
• Document patient assessment and visit details in EHR and notify Clinical Supervisor of findings
• Communicate care and updates with the care team
• Submit documentation of Evaluation and Follow-up visits according to the company policies
• Evaluate and create plan of care for patients independently

Qualifications

• Friendly, cheerful, and excellent customer service/bedside manners
• Minimum of 1-year experience seeing patients in the home setting
• Active California Occupational Therapy License

This is a Per Diem position. Annual Full Time Salary equivalent $143k, depending on experience.
