What You'll Do

• Evaluate patients and develop individualized plans of care.
• Provide treatment for strength, balance, mobility, gait, and functional independence.
• Address fall prevention and home safety.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with patients, families, and the interdisciplinary team.
• Complete timely and accurate clinical documentation.

Qualifications

• Active North Carolina Physical Therapist (PT) license.
• Strong clinical judgment, communication, and organizational skills.
• Dependable, compassionate, and comfortable working independently.
• Ability to manage a flexible schedule and travel to patients' homes in South Charlotte.

Home health, mobile therapy, or geriatric experience is a plus, but not required.
