About the Role

As a Nurse Practitioner, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

You will be responsible for building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.

Responsibilities

Conduct on-site visits and remote patient monitoring to identify potential health issues early Provide chronic care management and psychiatric collaborative care to patients Document patient visits and treatment plans using our streamlined documentation system Collaborate with our clinical team to develop individualized care plans for each patient Participate in ongoing training and education to stay up-to-date on the latest best practices and guidelines Requirements: Active NP or PA license in the state of New York 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Ability to work independently and as part of a team Benefits: Hybrid Role!
