Job Summary:

Carease Health - OH, a leading nationwide preventative healthcare company, is seeking an experienced Adult Nurse Practitioner to join our team in Napoleon, OH. As a key member of our clinical team, you will deliver high-quality preventative care to residents in skilled nursing facilities through a combination of on-site visits and remote work from home.

With a strong background in Geriatrics or Internal Medicine, you will build strong relationships with facilities, maintain a patient-centered approach, and be a collaborative team player.

Responsibilities:

• Provide comprehensive preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home
• Develop and implement individualized care plans, utilizing remote patient monitoring (RPM), chronic care management (CCM), and collaborative care management (CoCM) principles
• Conduct assessments, diagnose and treat acute and chronic conditions, and order and interpret diagnostic tests
• Collaborate with healthcare teams to ensure seamless care coordination and communication
• Maintain accurate and detailed records, utilizing a dedicated scribe team for efficient documentation
• Participate in quality improvement initiatives and contribute to the development of clinical protocols and guidelines

Requirements:

• Active Adult Nurse Practitioner license (in the state of practice)
• 2+ years of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
• Masters degree in Nursing or related field
• Ability to work independently and as part of a team
• Strong clinical judgment and critical thinking skills
