Role Summary

We are seeking a Registered Nurse to join our team on a PRN basis. You will perform nursing assessments, support care plans, and provide skilled nursing services in clients’ homes, working closely with our clinical leadership and caregivers.

Responsibilities

• Complete nursing assessments and contribute to client service plans
• Provide skilled nursing services as authorized and within your scope of practice
• Document visits accurately and on time, following agency and regulatory standards
• Communicate changes in client condition to the supervising nurse or clinical manager promptly
• Support and educate caregivers on care plan tasks
• Participate in care plan reviews and client coordination as needed

Qualifications

• Current, unrestricted Registered Nurse (RN) license in Georgia, or the ability to obtain one before start
• Current CPR certification
• Home health or home care experience preferred
• Strong assessment, documentation, and communication skills
• Reliable transportation and the ability to travel between client homes in the area
• Ability to pass a background check, drug screen, and any required health screenings
