  • Implement evidence-based therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to promote recovery and mobility.
  • Collaborate effectively with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to coordinate care plans and discharge strategies.
  • Document patient progress meticulously in electronic health records (EHR) systems while adhering to clinical documentation standards.
  • Manage discharge planning processes by preparing detailed care plans that facilitate seamless transitions from inpatient settings to home or community care environments.
  • Work with diverse patient populations, including those in geriatrics Educate patients and their families on treatment goals, home exercise routines, and community resources to support ongoing recovery outside clinical settings.
  • Skills Extensive knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques essential for effective treatment development.
  • Proven experience in outpatient clinic settings, inpatient hospital environments, or home health services with a focus on patient rehabilitation.
  • Strong skills in clinical documentation standards and electronic health records (EHR/EMR systems) management for accurate record keeping.
  • Ability to develop comprehensive treatment plans that incorporate discharge planning and address acute care needs.
  • Excellent client communication skills to explain treatment procedures clearly and foster trust with patients of all ages.
  • Experience working with geriatric patients and familiarity with home & community care practices enhances your ability to deliver holistic care.
  • Knowledge of specialized therapies such as trigger point therapy and techniques tailored for neurological or orthopedic conditions.
