You do not have to accept patients throughout the entire territory.

What You'll Do

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Help patients improve strength, balance, endurance, and mobility
• Provide therapeutic exercise and functional training
• Provide gait, transfer, and stair training
• Assess fall risks and home safety
• Help patients regain mobility following illness, surgery, injury, or hospitalization
• Educate patients, families, and caregivers
• Monitor patient progress and modify treatment plans
• Complete reassessments and discharge planning
• Collaborate with the interdisciplinary care team
• Complete timely clinical documentation using WellSky

What We're Looking For

• Active Maryland Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Reliable transportation
• Strong clinical and communication skills
• Ability to work independently in patients' homes
• Home Health Experience Preferred, but Not Required
• WellSky Experience is a Plus
