Cornerstone VNA is looking for a compassionate and skilled Physical Therapist (PT) to join our Home Care Rehabilitation team. Provide personalized, one-on-one care that helps patients regain mobility, improve function, and remain independent in the comfort of their own homes.

What You’ll Do

Patient Care

• Evaluate patients and develop individualized plans of care based on their needs and goals.
• Provide skilled physical therapy services in the home to improve mobility, strength, balance, function, and independence.
• Monitor progress and adjust treatment plans as needed to achieve meaningful outcomes.
• Educate patients and caregivers on treatment plans, exercises, safety, and strategies for continued progress.

Documentation & Care Coordination

• Complete timely and accurate documentation in the EMR.
• Communicate effectively with physicians, caregivers, and members of the interdisciplinary care team.
• Coordinate care and make appropriate referrals when additional services are needed.
• Ensure services and documentation meet agency, clinical, and regulatory standards.

Professional Growth

• Participate in quality improvement initiatives, trainings, and competencies.
• Stay current with evidence-based practices and incorporate new knowledge into patient care.
• Contribute to a collaborative, supportive culture focused on exceptional patient outcomes.

What You Bring

• Current Physical Therapist (PT) license in New Hampshire and Maine
• Graduation from an accredited Physical Therapy program
• 2+ years of experience preferred
• Home care experience preferred (minimum 1 year)
• Strong clinical judgment and problem-solving skills
• Excellent communication, organization, and documentation skills
• Ability to work independently and manage a flexible daily schedule
• A genuine passion for helping patients achieve greater independence
